Your DACA forms (both renewal and initial applications), require you to disclose your current physical address and mailing address. If you don't include your current address, your forms will be sent back by USCIS. Including valid addresses makes sure you receive all important mail and updates from USCIS regarding your application. Your new work permit card will also be mailed to your current mailing address.
